Creating a portal account does not establish you as a patient of the clinic. If you are seeking a primary care provider, please register with the provincial waiting list.

If the Patient Portal Account was not activated, a forgot password email will not be sent. Please be patient to receive the
Activation Email.
Each patient should only have one portal account.

Creating multiple accounts can cause issues with accessing your records, as the system only allows one portal account per patient profile.
Patients that share the same email can only create one Patient Portal Account. To link family members under your account for easier access, please contact our office directly by phone or email for administrative assistance.
For patients aged 14 and older, the patient must sign a consent form if a parent wishes to have access to their medical chart.
Without signed consent, a separate portal account must be created for the patient using a distinct email address.
